Presbyterian Centennial Care members should report harmful incidents (events) to Presbyterian Health Plan, Inc. Harmful incidents include but are not limited to abuse, neglect, exploitation, emergency services, death, environmental hazards, law enforcement intervention, or a missing person. Who should report a critical incident, and how? ● Any Presbyterian Centennial Care member ● Any personal care service agency for members who get home- and community-based services (per the New Mexico Human Services Department) ● Members who get self-directed benefit services ● Members who get behavioral health services ● Your representative Good faith incident reporting of or the allegation of abuse, neglect, or exploitation is free from any form of retaliation. To report a harmful incident, contact your Presbyterian Health Plan care coordinator. Make an appointment today through your myPRES account at www.phs.org . If you need help logging in to your account, call the number on the back of your Presbyterian ID card. PresRN Speak with a Presbyterian nurse if you are not feeling well and do not know what to do. PresRN phone calls are free and available 24 hours a day, 365 days a year.
