4 Presbyterian Health Plan has created a new pharmacy call center to help you with any questions you have about your pharmacy benefits or medications. The call center was created to give you a better service experience when calling about your pharmacy-related questions. You can contact the Presbyterian Pharmacy Customer Service Center at (505) 923-5200 or 1-888-977-2333 , and your call will be directed to one of our pharmacy specialists. The Presbyterian Pharmacy Customer Service Center is available Monday through Friday (except holidays) from 8 a.m. to 6 p.m. If you need to call outside of business hours, the Presbyterian Pharmacy Customer Service Center uses a voice response system to help you. Our voice response system can help you get assistance filling prescriptions at the pharmacy. We offer Spanish- and Navajo-speaking representatives to help you. We can also get you help in any other language you need. We understand how important your pharmacy benefits are to you. The call center is staffed with pharmacy specialists who can take care of your questions and concerns to provide you with a better pharmacy experience. Presbyterian’s care coordination services Presbyterian’s Pharmacy Customer Service Center Presbyterian Health Plan has a care coordination program for members. The program helps members who have special healthcare needs and complex health problems. Care coordinators work with you to create a plan to help you get the care you need. Care coordination can: • Help you get healthcare services you need • Help you with long-term illnesses and injuries • Help you communicate with your Healthcare Team • Make sure you have what you need when you leave the hospital • Help you and your caregiver(s) understand medications, diets, and other care needs • Work with you if you have mental or emotional needs or drug or alcohol problems • Help you with special needs, such as a transplant, high- risk pregnancy, or premature baby or if you need special services or equipment • Help you manage your diabetes, chronic obstructive pulmonary disease (COPD), heart disease, or heart failure For more details or for help with these services, please call our Presbyterian Customer Service Center at the number on the back of your ID card.
